CT HMIS - Password Reset Request Form

The CT HMIS helpdesk is covered 5 days a week from 9:00 AM to 5:00 PM. Using this form submits a ticket to our helpdesk.

The username used to log into the CT HMIS CaseWorthy Database.
Note: Usually the "CCEH (LIVE)" option is correct, as this is where you input real client data but some users may need to reset access to one of our databases such as the Learning Management System (LMS) database , Support database or Development database.
