We now have a new process for handling duplicate ID’s. Going forward, if you would like to report a duplicate client/request a merge in HMIS, we ask that you fill out and submit a Client Merge Request form on cthmis.com. Once submitted, we will receive the request on our help desk. The user submitting and the HDC of each Organization who has ever worked with this client will be notified when the merge request has been processed.
