Request a Report

ServicePoint contains a variety of reports you can run on your agency and clients. While some reports are already built into ServicePoint under the "Reports" tab, others are only available through the Advanced Reporting Tool (ART). If you do not have an Art license you can have CT HMIS support run them for you. Simply click on the "Request Report" link under each report and fill out all the form fields on the next page.

If you'd like to be able to order an ART license so you can run a report at any time without our assistance, please download and complete the order form.

You can Request a Custom Report. Please provide a detailed request description and fill out any fields you think you need to get the required data.

Report Packs

What is a Report Pack? - A Report pack is a group of individual reports that we recommend you run on a regular basis for your program to ensure your data is as good as possible. Reports listed in each Report Pack are specifically picked because they work with the data and workflow your program should be following.
