The Department of Social Services (DSS), Connecticut Coalition to End Homelessness (CCEH) and Nutmeg Consulting LLC (Nutmeg) worked together to develop a Housing First for Families (HFF) Program report generated from HMIS. Now that the H:MlS report and client assessment are complete, HFF coordinators can enter their client information into H:MlS and pull quarterly reports in accordance with the DSS contract, Part I, Section 1and Section K.
Nutmeg Consulting has held two training sessions for HFF coordinators on the HFF data entry into HMIS and generating quarterly reports from H:MlS to email to DSS. All HFF regional programs were informed of this training by Nutmeg and reminded by their DSS contract representative. The expectation is that each region will be able to generate and analyze the E725 Report which is the HFF program report on a quarterly and annual basis; as well as the A 108 Report, which is the Data Quality Report. Both reports can be generated as much as the HFF program wants in order to check up on the data quality and completeness of information going in to HMIS. The E725 Report must be generated and emailed to DSS in a pdf file on a quarterly and annual period.
The expectation is that all HFF regional programs begin entering data by January I. 2012. As of this date, any open and active cases should be entered in to HMIS. DSS is not expecting m:F regional programs to go back and enter any client information prior to January L 2012. By beginning on 1/1/12. we will be able to capture the fmal full year of HFF data and use it to determine the successes and challenges of the program. DSS expects only one quarterly report per region. If an HFF program has subcontractors, they must work together to complete a data sharing agreement and select one person to run the report and review it for data quality each quarter. If subcontractors did not attend the required training, it is the HFF contractor’s responsibility to ensure that data is entered into HMIS until additional training can be coordinated and said HFF contractor should put in writing to its identified DSS liaison their action plan to address HMIS data entry for their region, until the entire region.
